Community First Sports Club (CFS) provides scholarships for registration fees to children, who without financial assistance, would be unable to participate in CFS sports programs.
The CFS Scholarship Board, comprised of the Director of Operations and the Director of Coaching, will consider all applications received by the application deadline; scholarship requests must be submitted to the CFS no later than one (1) week prior to the regular registration deadline.
The amount of the scholarship awarded (if any) may be a partial or full scholarship depending on the number of applicants and the assessment of needs.
Approval of scholarship does not register the participant in the program. Parent / Guardian must still register online or in person for the CFS program for which the scholarship was awarded.
No guarantee of assistance is implied in the application.
REQUIREMENT FOR ELIGIBILITY
Athlete must be eligible age to participate in CFS program.
Parent / Guardian commits that the athlete will attend a minimum of 90% of all scheduled practices and games for competitive and development teams and 80% of all practices and games for recreation programs.
Parent / Guardian agrees to volunteer 5 hours, per scholarship recipient per season. Hours will support CFS related activities and must be completed no later than 1 month after the completion of the season.
Applications must be completed by a parent, guardian, or head of household with all requested information provided.

A parent, guardian, or head of household must complete the application, with all requested information provided:
State or Federal assistance documentation
Letter of recommendation from school, community, or social services
Letter of hardship
All items on the Scholarship Terms and Conditions must be initialed and the form signed and dated.
Scholarship approval will be based on the verification of financial need, number of scholarship applicants, and space availability.
All on-time applicants will be notified of scholarship status prior to the registration deadline.
Please provide all information required to help CFS determine qualifications. Scholarship consideration will be given to families that meet the following criteria:
Provide a copy of your IRS form 1040 from the most recent tax year and current paystubs
Provide documentation of participation in assistance programs such as: Food Stamps, Medicaid, SSI, Foster Care, WIC, etc.
Provide recommendation by school representative, social worker, youth community center, or other social service representative.
Provide a written statement of immediate financial hardship explain the current situation.
CFS recognizes that a family may not be receiving formal assistance from the programs mentioned above, yet financial assistance may still be needed to participate in CFS programs. In these instances, the CFS Scholarship Board will consider the financial hardship statement to determine scholarship eligibility. Please provide any supporting documentation that may support the facts in your financial hardship statement.

SCHOLARSHIP TERMS AND CONDITIONS
“I”, “me” and “my” refer to the adult scholarship applicant.

_____ 3. I understand that members of the Community First Sports Club Scholarship Commitee consider each scholarship application on a case-by-case basis.
_____ 4. I understand that no guarantee of assistance is implied by this application and scholarships are awarded if funds are available.
_____ 5. I understand that I am responsible for any equipment and uniforms required for my child’s participation.
_____ 6. I understand that scholarship money will not be paid to the individual recipient, nor will any money be refunded to the individual recipient.

I understand that if any information provided during the scholarship application is deemed inaccurate, Community First Sports Club may immediately terminate my child’s privilege to benefit from the scholarship program, and in the case any information was intentionally false, I will repay CFS the full value of any scholarship awarded.
_____ 8. I understand that if a scholarship is awarded to my child or multiple children, I am required to volunteer 5 hours, per scholarship recipient, per season. Failure to satisfy this condition will disqualify me, my child(-ren), and my immediate family from being considered for another scholarship for 12 months.

Community First Sports Club will use the information on the application only to decide if you child qualifies to receive a partial or full scholarship for eligible teams or programs. Confidentiality will be maintained at all times. Applicants are guaranteed that personal finances will not be discussed outside of the CFS Scholarship Committee. Coaches, assistant coaches, or volunteer helpers will not be informed of a participant’s financial or scholarship status.
